Assisted Home Health Care
Personal Care Assistants (PCAs) are provided by Assisted Home Health Care for seniors in need. PCAs help with the activities of daily living that keep your loved one safe in their home. They assist clients with eating, bathing, walking from room to room, and running errands. PCA workers keep clients safe in their homes by being there to help with daily tasks.
245D Waiver Services provide support and training for adults living in their own homes. Our services and supports aide individuals to live independently in their communities, and include household management, community participation, adaptive skills, health, safety and wellness. In addition, our services can be provided in public community settings, and either in person or remotely.
People with disabilities often need support to live successfully in the community. Affordable housing isn’t always enough; challenges such as mental and developmental disabilities can make it difficult to find housing, budget, and understand the rules of a lease. But with the right support and knowledge in housing, individuals can succeed.
Integrated Community Supports (ICS) include services that provide support and training in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person’s living unit or in the community.
